Panel consent information

Survey participation is always voluntary.

• As a participant, you may refuse to answer any survey question or stop a survey at anytime without penalty.
• As an APP member, you may opt out of the panel at anytime. Your decision will not affect your future relationship with CHRR at The Ohio State University.
• Individuals who repeatedly complete the same survey or display other fraudulent behavior will have their survey submissions suspended for review and/or disqualified from the corresponding study.

There are no costs or incentives for joining the panel.

• As an APP member, incentives may be offered for future survey participation when your profile information matches the criteria of a research study.
• Time commitment and incentives vary with each research study.

CHRR at The Ohio State University has a comprehensive security program for the data protection of your profile information.

• As an APP member, you may activate an optional APP account that provides you with access to your profile information and additional opt in/out features.
• Upon joining the panel, you agree to prohibit others from accessing your account information or survey invitations and will notify the APP of any unauthorized use.
• Individuals attempting to create multiple APP membership profiles will be suspended for review and/or disqualified from the APP.

You may receive APP communications, such as newsletters and other APP-related research news.

• As an APP member, you may opt out of additional communications in your account preferences or by contacting us directly. Please see our FAQs page for information about accessing your panel member account for opt in/out features.

The privacy of all panel members is protected.

• We do not share or sell any panel member profile information, including your contact information.
  • Third-party vendors, such as Tango Card, do not have access to your profile information.
• Researchers do not have access to your profile information. They only receive the resulting de-identified data from a survey.
• Limited CHRR staff have access to member profiles for panel maintenance, survey announcements, and incentive payments. ALL staff with access to profile information undergo regular and rigorous data security training.
• In the case of a data breach, APP members will be notified directly by CHRR at The Ohio State University.

We make every effort to keep your information confidential. However, there may be circumstances where this information must be released. For example, personal information regarding your participation in a study may be disclosed if required by state law. Also, your records may be reviewed by the following groups (as applicable to the research): OSU Office for Human Research Protections or other federal, state, or international regulatory agencies; OSU Institutional Review Board or OSU Office of Responsible Research Practices.

For individuals who have chosen text messaging as a contact mode in their profile information, standard service messages include the following:

• Service description: "Text JOIN to 69493 to periodically receive surveys sent by SMS"
• Fee disclosure: "Message and data rates may apply"
• Recurring message disclosure: "Message frequency varies"
• Customer care information: "Text HELP for help"
• Stop SMS messages, but stay in the panel: "Text STOP to remove text messaging"
• Opt out of panel instructions: "Text PANELOUT to cancel panel membership"
